You will assist clients with their daily routines, monitor health status, and ensure their comfort and safety while fostering independence and dignity. Whether working with seniors, individuals with disabilities, or those recovering from illness, your efforts will promote well-being and enhance quality of life. Duties Provide assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Observe and document patient conditions accurately, including vital signs, medication adherence, and behavioral changes, utilizing Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support clients with specialized care needs including Alzheimer's care, dementia management, memory care, hospice support, and working with individuals with developmental disabilities or disabilities. Prepare nutritious meals and assist with feeding to meet dietary requirements while maintaining safety standards. Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall health and comfort. Use medical terminology appropriately when documenting patient records or communicating with healthcare teams. Job Details Approximately 36 hours per week

9:00 AM - 3:30 PM

Saturday or Sunday:
