We are specifically seeking candidates who can converse in one of the following languages: Hindi, Gujrati and Punjabi as our clients need to converse in these languages. This is a live-in position. You will be with the client 6-7 days per week including overnight. Join our compassionate healthcare team as a Home Health Care Aide, where your dedication and caring nature will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized in-home care, assisting individuals with their daily needs and promoting independence. Your energetic approach and attention to detail will ensure each client receives safe, respectful, and comprehensive support tailored to their unique circumstances. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, or those recovering from illness, fostering a warm and trusting environment. Duties Deliver compassionate assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' conditions regularly, documenting vital signs such as blood pressure, pulse, temperature, and respiration accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Assist with medication administration following prescribed guidelines while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice care, or working with individuals with developmental disabilities or disabilities Operate medical equipment including Hoyer lifts for safe patient transfers and perform catheter care when necessary Prepare nutritious meals and assist with meal planning to promote health and well-being Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to support overall health and safety at home Document medical records accurately and maintain detailed medical documentation for each client's care plan Collaborate effectively with healthcare professionals using EMR/EHR systems to ensure seamless communication of patient progress and updates Provide emotional support and companionship to foster a positive environment that enhances patients' quality of life Assist with hospice or end-of-life care when required, respecting the dignity and wishes of each individual Qualifications - We are seeking Hindi speaking or Punjabi Speaking aides at this time. Proven experience in home health care or long-term care environments; experience in assisted living or nursing homes is highly valued Strong knowledge of medical terminology and clinical procedures such as vital signs measurement, catheter care, medication administration, and CPR certification Experience working with people with disabilities or special needs including dementia or Alzheimer's disease is preferred Familiarity with EMR/EHR systems for accurate documentation of patient information Ability to lift patients safely using Hoyer lifts or other assistive devices; physical stamina for long shifts is essential Excellent patient observation skills to identify changes in condition promptly Compassionate communication skills to build trust and rapport with clients from diverse backgrounds Ability to work independently while following detailed care plans; flexibility for live-in employment or varied schedules is advantageous Knowledge of HIPAA regulations ensuring confidentiality of all patient information Valid driver's license and reliable transportation are often preferred for community-based visits Join us in delivering exceptional home health services that empower individuals to live safely and comfortably at home.
