Pittsburgh, PA, West Mifflin, North Braddock, Homestead Available shifts: 40 + hours/week Weekend Blocks Saturday 7am- Monday 7 am- Most Need Overnights 11p-7a Evenings 3p-11p Mornings 7a-3p

FUNCTION

Provides direct care and personal support to individuals with intellectual and developmental disabilities in their own homes. Complies with the Department of Human Services standards. Ensures the health, safety, and welfare of the individuals supported reports to the Program Manager.

RESPONSIBILITIES

Advocates for the independence of the individual's by teaching skills in communication, decision-making, and social skills Completes tasks and assignments as directed. Assist with daily living activities, personal hygiene, meal prep/meal planning, and medication management. Communicates and resolves program and individual concerns with the Program Manager regularly. Participates in general staff meetings. Participates in various committees and workgroups as requested or assigned. Serves as a model of appropriate language, dress, and behavior. Ensures effective communication with the individuals supported, families and other people involved in the provision of supports for the individual as directed. Accurately documents ADL's, progress, and incidents Performs other related duties as assigned. Maintains a clean and safe environment.
