Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques Monitor patients' vital signs and observe changes in health status to ensure timely intervention and communicate findings to healthcare professionals Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation Administer medications accurately following care plans and ensure compliance with HIPAA regulations to protect patient privacy Support clients with memory care needs, including dementia or Alzheimer's care, utilizing specialized techniques for safety and engagement Maintain detailed documentation of patient care activities, observations, and medication administration in accordance with facility policies Provide transportation assistance for appointments or errands using safe patient transfer methods while ensuring client safety at all times Experience Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred Demonstrated ability to observe patients closely and respond appropriately to changes in condition or behavior Knowledge of infection control practices including cleaning protocols and maintaining a safe environment for residents or clients Hands-on experience supporting individuals with developmental disabilities, neurodevelopmental disorders, or intellectual disabilities support is advantageous Familiarity with care plans, patient monitoring techniques, and medication administration procedures Certification in CPR and First Aid is required; additional certifications such as HIPAA training or hospice care experience are a plus Strong communication skills for effective client interaction and collaboration with healthcare teams Join us to be part of a compassionate team that values your dedication.
