Job Description

Job Overview As a Personal Care Assistant, you will play a vital role in enhancing the quality of life for individuals in need of assistance. The Personal Care Assistant (PCA) provides essential support and assistance to PACE participants in both the PACE center and in participant homes, ensuring their daily needs are met with dignity and respect. This role focuses on enhancing the quality of life for our PACE participants by delivering personal care services, promoting independence, and maintaining a safe and supportive environment. Responsibilities Assist participants with activities of daily living (ADLs) including bathing, dressing, grooming and toileting. Provide mobility assistance, including transfers, positioning, and ambulation support. Support participants with meal preparation and feeding, ensuring adherence to dietary guidelines. Monitor and report any changes in participants' health status to the appropriate medical staff. Participate as a member of the IDT and contribute to the participant care plan process. Ensure a clean and safe environment by maintaining hygiene and sanitization standards. Follow all infection control and safety procedures according to the program guidelines. Adhere to HIPAA and confidentiality regulations when handling participant information. Engage participants in meaningful activities to promote socialization and mental well-being. Provide companionship and emotional support to enhance participant quality of life. Respect participant preferences and cultural backgrounds in all care activities. Work closely with interdisciplinary teams, including nurses, therapists, and social workers, to coordinate participant care. Communicate effectively with participants, families, and caregivers to address concerns and provide education on care plans. Document care services provide and report observations to appropriate team members. Other duties as assigned. Experience Ideal candidates will possess a combination of the following skills and experiences: High school diploma or equivalent . Certificate from an approved CNA/PCA training course in the state of Pennsylvania required Current American Heart Association BLS Certification Minimum one year of experience working with a frail or elderly population required. Valid unrestricted Driver's license & proof of insurance Minimum of one year of experience as a CNA/PCA. Exceptional customer service skills. Results-oriented; ability to handle multiple, time-sensitive projects and prioritize accordingly, while focusing on delivering required outcomes Commitment to demonstrating the highest standard of ethical conduct and integrity; strong willingness to continually embrace personal and professional development Familiarity with patient observation techniques and monitoring vital signs. Background in caregiving for individuals with developmental disabilities or Alzheimer's/dementia care is highly desirable. Knowledge of HIPAA regulations to ensure client confidentiality is maintained. Experience in meal preparation and cooking for individuals with dietary restrictions or preferences. Ability to perform heavy lifting as required when assisting clients with mobility challenges. Prior experience in social work or direct support roles is a plus.
