Committed to promoting dignity, comfort, and quality of life while assisting individuals through end-of-life care. Skilled in delivering personalized care that supports independence, safety, and overall well-being in a home environment. Key Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting, while maintaining dignity and respect Support safe mobility and transfers using assistive devices such as Hoyer lifts Maintain a clean, safe, and organized living environment Monitor and document patient condition and care provided in compliance with medical documentation standards and HIPAA regulations Assist with basic medical procedures, including catheter care and hospice support, under professional supervision Collaborate with nurses and healthcare teams to ensure coordinated, high-quality care Accurately utilize EMR/EHR systems for documentation and communication Qualifications Certified Home Health Aide (HHA) license (state-approved) Experience in home health care, hospice, assisted living, or nursing home settings Knowledge of medical terminology and patient care procedures Strong observation skills with the ability to respond to changing patient needs CPR certification required (additional certifications in hospice or catheter care preferred) Familiarity with EMR/EHR systems and medical records management Excellent communication skills with a compassionate, patient-centered approach Ability to work independently while following established care plans

Pay:

$21.00 - $25.00 per hour
