In this vital role, you will provide personalized assistance to clients with diverse needs at the end of their lives. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' conditions regularly, documenting vital signs and changes in health status accurately Operate Electronic Health Record (EHR) systems to record medical documentation and update patient records efficiently Provide specialized care including Alzheimer's and dementia care, hospice support, catheter care, and memory care as needed Use assistive devices like Hoyer lifts safely to prevent injury during transfers and mobility assistance Requirements Proven experience in home health aid Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus Ability to work with individuals with patience and empathy Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate record keeping Strong understanding of HIPAA regulations to ensure patient confidentiality at all times Physical ability to perform caregiving duties including lifting using Hoyer lifts, assisting with transfers, and providing mobility support Excellent communication skills for working effectively with clients, families, and healthcare teams Flexibility to work live-in shifts or part-time schedules based on client needs Join us in delivering compassionate services.
