Job Description

Job Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals needing support at home. In this vital role, you will provide personalized assistance to patients with diverse needs, including seniors, individuals with disabilities, and those recovering from illness or managing chronic conditions. Your energetic and proactive approach will help promote independence, safety, and well-being in a comfortable home environment. This paid position offers an opportunity to develop your caregiving skills while making a positive impact in the community. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or wheelchairs. Monitor and record vital signs including blood pressure, pulse, respiration, and temperature to track health status accurately. Administer medications following medical documentation protocols and ensure compliance with HIPAA regulations to protect patient privacy. Support patients with memory care needs related to Alzheimer's disease and dementia by implementing specialized care techniques and ensuring a safe environment. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a hygienic living space to promote overall health. Document patient progress and medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems accurately and timely. Communicate effectively with healthcare teams about patient status, changes in condition, or concerns related to hospice or long-term care needs. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to observe patient conditions closely through patient observation skills. Experience working with individuals with developmental disabilities or special needs is advantageous. Knowledge of medical terminology, medication administration procedures, and medical documentation practices is essential. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as catheter care or hospice care are a plus. Familiarity with EHR/EMR systems for maintaining accurate medical records and documentation. Long-term care experience or working with patients requiring memory care or dementia-specific support will strengthen your application.
