Job Description
Job Overview We are seeking a compassionate and dedicated Hospice Home Health Aide to provide exceptional personal care and support to patients in their homes during end-of-life care. This vital role involves assisting patients with daily activities, ensuring their comfort, and maintaining dignity while adhering to home healthcare regulations and privacy standards. The ideal candidate will bring energy, empathy, and a strong commitment to enhancing the quality of life for our clients, primarily focusing on geriatric and hospice patients. Responsibilities Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary Monitor and document patient condition through patient observation, vital signs measurement, and medical documentation in electronic health record (EHR) systems Support medication administration under supervision, ensuring proper handling and adherence to medication schedules Provide specialized care for patients with dementia, Alzheimer's disease, or other memory care needs, including behavioral management and environmental safety Assist with catheter care, wound care, and other medical procedures following home healthcare protocols Maintain patient confidentiality in compliance with HIPAA regulations and ensure a safe environment by adhering to home health care assistance standards Support families by providing education on caregiving techniques, meal preparation, light cleaning, laundry, and overall home safety Qualifications Proven experience working with individuals with disabilities, developmental disabilities, or in assisted living environments Prior experience in geriatrics or hospice care is highly preferred Knowledge of medical terminology, patient transfer methods, and basic life support (BLS) or CPR certification required Familiarity with EMR/EHR systems for accurate medical records management Ability to perform heavy lifting safely and handle physically demanding tasks such as moving patients or equipment like Hoyer lifts Strong communication skills for effective client interaction and family support Valid certification in first aid and CPR; additional certifications related to hospice or home health are advantageous Experience working within home health agency regulations and understanding of home & community care standards Join us in delivering compassionate hospice care that truly makes a difference.
