Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, accurately documenting vital signs and health status in electronic health record (EHR) systems Support medication administration and ensure adherence to prescribed treatment plans under supervision Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques Perform catheter care, wound care, and assist with hospice or end-of-life care as needed Help with light housekeeping duties such as cleaning, laundry, meal preparation, and maintaining a safe environment Follow home healthcare regulations and HIPAA guidelines to protect patient privacy and confidentiality Qualifications Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred Knowledge of medical terminology and medical documentation practices Certification in CPR (Cardiopulmonary Resuscitation) and Basic Life Support (BLS) required; additional certifications such as First Aid are a plus Ability to lift heavy objects safely using equipment like Hoyer lifts when necessary Experience working with geriatric patients or individuals with disabilities or developmental disabilities highly desirable Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation Strong communication skills to effectively interact with patients, families, and healthcare teams Valid driver's license and reliable transportation preferred for travel between patient homes Join us if you're passionate about providing compassionate home healthcare that empowers individuals to live comfortably and safely.
