Part-Time (10-30 hours per week) Job Overview CareBridgeTN is seeking compassionate, dependable, and motivated Community Living Specialists to support adults with intellectual and developmental disabilities (I/DD) in achieving greater independence and full participation in their communities. As a Community Living Specialist, you will provide person-centered Community Habilitation services that help individuals build everyday life skills, increase independence, develop meaningful relationships, and actively engage in community life. Services are delivered in community settings based on each person's Individual Support Plan (ISP) and personal goals. Tennessee's HCBS waivers emphasize person-centered planning, independence, and community integration. Essential Duties & Responsibilities Provide person-centered Community Habilitation services according to each individual's ISP. Assist individuals in developing independent living skills, including communication, decision-making, self-advocacy, money management, meal planning, and personal organization. Support participation in community activities such as volunteering, recreation, shopping, fitness, education, and social events. Encourage community inclusion while promoting individual choice, dignity, and self-determination. Assist with transportation to approved community activities using a personal or agency-approved vehicle, when applicable. Document services accurately and complete required electronic documentation in a timely manner. Maintain confidentiality and comply with HIPAA, DIDD, TennCare, and CareBridgeTN policies. Recognize and report health, safety, behavioral, or environmental concerns promptly. Participate in required meetings, supervision, and ongoing training. Build positive relationships with individuals served, families, Support Coordinators, and community partners. Promote a safe, respectful, and inclusive environment at all times. Qualifications High school diploma or GED required. Must be at least 18 years of age. Valid driver's license with reliable transportation. Ability to pass required background checks and meet Tennessee DIDD employment requirements. Current CPR and First Aid Certification (or willingness to obtain upon hire). Excellent communication and interpersonal skills. Ability to work independently while maintaining professionalism and accountability. Experience supporting individuals with intellectual or developmental disabilities is preferred but not required. Ability to lift up to 30 pounds and assist individuals with routine community activities as needed. Preferred Qualifications Experience working in Community Habilitation, Personal Support Services, Supported Living, or other HCBS programs. Knowledge of person-centered practices and positive behavior support. Familiarity with Tennessee DIDD or TennCare waiver services. Flexible availability, including evenings or weekends.
