In this vital role, you will provide compassionate assistance to clients in their homes, helping them maintain independence, dignity, and comfort. Your energy, attention to detail, and genuine care will foster a safe and nurturing environment for those you serve. This position offers an opportunity to work closely with clients of all ages, including seniors, individuals with disabilities, and those recovering from illness or injury. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals Support medication assistance following prescribed instructions while maintaining compliance with HIPAA regulations for patient confidentiality Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure seamless communication among care teams Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment Experience Prior experience in home health care or assisted living environments preferred; experience in nursing homes or long-term care settings highly valued Familiarity with medical terminology, medical documentation practices, and working within HIPAA guidelines is essential Hands-on experience with patient observation Knowledge of working with individuals with developmental disabilities or disabilities in general enhances your effectiveness in this role Demonstrated ability to assist with ADLs and provide compassionate senior care or home & community care services Ability to work independently in live-in employment settings or flexible schedules that meet client needs Join us to deliver exceptional home health services that promote independence and improve quality of life.
