Available shifts:

Saturday and Sunday 7:00am- 10:00pm Job duties include, but are not limited to: Provide personal assistance and health related services to patients in home and at facility according to the Individualized Service Plan. Ensure a clean, safe, and comfortable environment that promotes respect for patients' privacy and property. Appropriately report changes to certify continuity of care. Maintain positive, supportive, and respectful communication with the patient, their family, and agency personal. Performs other duties as assigned.

Minimum qualifications include:

High School diploma or equivalent. 1 + years of experience as CNA or PCA is highly preferred but not required. Experience using patients lifts such as the Hoyer, Sit to Stand and Gait belt is preferred but not required. Able to read, write and comprehend care plans and other tasks in English. Excellent verbal, written, interpersonal and medical documentation skills. Must be comfortable working in client's home. Ability to lift 50lbs or more.
