Job Description

Job Summary Join our dedicated team as a Personal Care Assistant, where your compassionate support and proactive care will make a meaningful difference in the lives of individuals requiring assistance with daily activities. In this vital role, you will provide personalized care to promote independence, safety, and well-being in various settings including assisted living facilities, home environments, and community programs. Your energetic approach and attention to detail will help create a nurturing environment that empowers clients to thrive. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and dignity. Monitor patients' health by observing vital signs, reporting changes, and maintaining accurate documentation in compliance with HIPAA regulations. Support medication administration and manage care plans tailored to individual needs, including Alzheimer's, dementia, autism, or developmental disabilities. Prepare nutritious meals and assist with meal planning to promote healthy eating habits aligned with dietary requirements. Operate mobility aids such as Hoyer lifts and assist with transfers to ensure safe movement for residents with mobility challenges. Maintain a clean and safe environment by performing light housekeeping tasks including cleaning patient areas and ensuring infection control standards are met. Collaborate with healthcare professionals, social workers, and family members to deliver comprehensive patient-centered care. Skills Proven experience in home health, hospice care, long-term care, or working within assisted living or nursing home settings. Strong knowledge of patient observation techniques, vital signs monitoring, and medication administration. Certified in CPR and First Aid; familiarity with HIPAA regulations for patient privacy is essential. Experience working with individuals with disabilities such as autism or developmental delays; knowledge of dementia and memory care is highly valued. Ability to perform heavy lifting safely and operate mobility equipment like Hoyer lifts effectively. Excellent communication skills for engaging with patients, families, and interdisciplinary teams.
