Job Summary We are seeking a dedicated and proactive Home Health Community Liaison to serve as a vital link between our agency and the community. The ideal candidate will possess excellent communication skills, a strong understanding of home healthcare services, and experience working with diverse patient populations. In this role, you will promote our services, build relationships with community partners, and ensure clients receive comprehensive support within their home environments. Duties Develop and maintain relationships with community organizations, healthcare providers, and referral sources to promote agency services. Educate patients, families, and community stakeholders about available home health options, including hospice, dementia care, and geriatric support. Assist in coordinating patient care plans by facilitating communication between clients, families, and healthcare teams. Conduct outreach activities such as presentations, health fairs, and informational sessions to increase agency visibility. Ensure compliance with home healthcare regulations and maintain thorough documentation of community engagement efforts. Support patients with understanding their care options while respecting confidentiality and HIPAA guidelines. Collaborate with internal teams to ensure seamless transition of care from hospital or nursing facilities to home settings. Experience Proven experience working in assisted living facilities, nursing homes, or home healthcare agencies. Familiarity with medical terminology, patient observation techniques, and documentation systems such as EMR (Electronic Medical Records). Prior experience assisting with Activities of Daily Living (ADLs), medication administration, catheter care, vital signs monitoring, and end-of-life care is highly desirable. Knowledge of home health care regulations, HIPAA compliance, and patient confidentiality standards. Experience working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease enhances suitability for this role. Strong communication skills tailored to engaging geriatric patients and their families effectively. Ability to perform light physical tasks including heavy lifting or patient transfers using equipment like Hoyer lifts when necessary.
