PSL Cares Employee Assistance Program Highlights of the Care Coordinator| Lead Home Health Aide position: Coordinate caregiver schedules and resident service requests Ensure residents receive exceptional care and personalized support Mentor, train, and support Home Health Aides and caregivers Assist with staffing, scheduling, documentation, billing support, and service coordination Conduct resident satisfaction follow-ups and quality assurance visits Help maintain continuity of care and service excellence Provide direct resident support when needed, including: Personal care assistance Medication reminders Transportation and escort services Grocery shopping and errands Light housekeeping and laundry Companion care and social engagement Pet care and dog walking What We're Looking For Home Health Aide certification or caregiving experience preferred Previous scheduling, staffing coordination, or leadership experience preferred Experience working with older adults strongly preferred Strong organizational, communication, and customer service skills Ability to manage multiple priorities in a fast-paced environment Valid driver's license and reliable transportation required Positive attitude, strong work ethic, and passion for serving seniors
