This vital role involves assisting patients with daily activities, monitoring their health, and providing emotional support in various care settings such as assisted living, nursing homes, or home health environments. Responsibilities Provide personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately and promptly Assist with medication administration following HIPAA guidelines and care plans established by healthcare professionals Support patients during meal preparation, feeding, and ensure proper nutrition tailored to individual needs Utilize specialized equipment such as Hoyer lifts or resident's to safely transfer individuals with mobility challenges Maintain a clean and safe environment through regular cleaning duties aligned with health standards and infection control protocols Collaborate with nursing staff, social workers, and other care team members to develop and implement effective care plans Skills Proven experience in group home settings, assisted living facilities, or home & community care environments Knowledge of patient observation techniques, vital signs measurement, and basic nursing skills including CPR and first aid certification Familiarity with dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or autism spectrum disorder Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts Excellent communication skills for working effectively with patients, families, and interdisciplinary teams Strong understanding of HIPAA regulations to ensure patient confidentiality and data security Experience in medication administration, care plan adherence, and working within long-term care or nursing home settings Embark on a rewarding career where your compassion meets your expertise.
