Social Worker - SSW, CSW, LCSW PRN | Home Health Tooele County Area -
Schedule:
Monday-Friday, 8am-5pm Position Summary CNS is seeking a compassionate PRN Social Worker to support Home Health and Hospice patients throughout the greater St George Utah area. This role offers meaningful 1:1 care, greater autonomy, and the opportunity to work with a mix of adult and some pediatric patients.
Compensation & Benefits SSW:
Starting at $24.38+, We offer more hourly based on experience, education and work history.
CSW:
Starting at $26.44+, We offer more hourly based on experience, education and work history.
LCSW:
Starting at $33.65+, We offer more hourly based on experience, education and work history.
Mileage:
$0.57/mile reimbursement Why Home Care at CNS? Better work-life balance than facility-based shifts and patient ratio 1:1 patient care with strong clinical support Autonomy with fewer interruptions Meaningful relationships built in the home environment Key Responsibilities Conduct psychosocial assessments for Home Health and Hospice patients and families Provide counseling and emotional support to patients and caregivers coping with illness, end-of-life issues, and grief Coordinate community resources including financial assistance, housing, transportation, medical equipment, and support services Assist with advance care planning, including advance directives and end-of-life wishes Facilitate communication between patients, families, and the interdisciplinary care team Advocate for patient needs and coordinate discharge planning and care transitions Provide crisis intervention and support during acute medical or psychosocial situations Coordinate care with physicians, nurses, therapists, CNAs, chaplains, and leadership Educate and support patients, families, and caregivers on disease processes, coping strategies, and available resources Facilitate family meetings and care conferences as needed Maintain accurate documentation per CMS, state, and CNS standards Participate in interdisciplinary team meetings and quality improvement initiatives Adhere to safety, infection control, and HIPAA guidelines Qualifications LCSW License or Master's degree in Social Work (MSW) from a CSWE-accredited program Current Utah Certified Social Worker (CSW) license in good standing or SSW License Hospice, home health, palliative care, or medical social work experience preferred Pediatric experience a plus Knowledge of community resources, insurance systems, and Medicare/Medicaid regulations Strong assessment, counseling, and crisis intervention skills Excellent communication and interpersonal skills Ability to work independently and manage field-based caseload Valid driver's license and reliable transportation BLS certification preferred or able to obtain within 30 days of hire Physical Requirements Ability to drive throughout assigned service areas and surrounding communities Ability to sit, stand, or walk for extended periods Ability to navigate stairs and various home environments Physical stamina to conduct home visits throughout the day This job summary outlines the general nature and level of responsibilities and is not an exhaustive list of all duties or skills required.
