Job Description

Job Summary We are seeking a compassionate and energetic Personal Care Assistant to provide essential support and care to individuals in various settings, including assisted living, home health, and community care environments. In this role, you will assist clients with daily activities, monitor their health and well-being, and ensure a safe, comfortable environment. Your dedication will help enhance quality of life for those with diverse needs, including seniors, individuals with disabilities, or those experiencing memory loss or developmental challenges. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts. Monitor patients' vital signs and observe for any changes in health status, reporting findings accurately to healthcare professionals. Prepare nutritious meals and assist with meal planning in accordance with care plans and dietary restrictions. Administer medications following prescribed schedules while adhering to HIPAA regulations to ensure confidentiality. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are met. Support patients during outings or community activities to promote social engagement and independence. Follow detailed care plans tailored to each individual's needs, including specialized dementia or Alzheimer's care, autism support, or hospice care. Document patient observations, progress notes, and incident reports meticulously for ongoing care coordination. Use patient monitoring tools and techniques to ensure safety during activities and rest periods. Collaborate with healthcare teams, social workers, and family members to deliver comprehensive care aligned with each resident's goals. Experience Prior experience working in assisted living facilities, nursing homes, group homes, or home health environments is highly preferred. Knowledge of patient observation techniques and vital signs monitoring is essential. Experience assisting with medication administration and understanding care plans is advantageous. Familiarity with HIPAA regulations ensures confidentiality and proper handling of sensitive information. Hands-on experience with mobility aids such as Hoyer lifts or transfer devices is beneficial. Demonstrated ability to work compassionately with individuals who have developmental disabilities, dementia, Alzheimer's disease or autism spectrum disorder. Previous roles involving caregiving for seniors or individuals requiring long-term care support will be considered a strong asset.
