Job Description
Duties and Responsibilities:
Direct Support Professional Team Leader will serve as the assistant to the Residential Manager. His/her duties will include, but are not limited to, the following:- Ensure that staff is providing health and safety and proper supervision at all times.
- Ensure that staff are providing active treatment, interaction and person centered activities as scheduled
- Ensure that staff is attending the company and outside recreation events as scheduled.
- Ensure that staff community outings are planned on a weekly basis if possible (delegate to staff) field trips and vehicle request are due prior to community outings.
- Ensure that individuals' monthly weights are taken by the 1 st and the 15th of each month.
- Ensure that all assigned reports are completed as scheduled and reports should be studied prior to report presentation.
- Ensure that all progress notes, behavior data sheets, overnight sleep charts, and all incident reports (as needed) are completed by the end of each shift.
- Ensure that correct and prompt staff disciplinary actions take place when necessary and are reported to the residential manager. (Staff tardiness, attendance, and client issues, on -call issues.)
- Ensure that all progress notes, data collection sheets are pulled from and replaced in individuals' books by the 1st of each month.
- Ensure that staff are documenting as well as implementing individuals' programs, completing data collection and initialing for training daily.
- Check all individuals' books daily and initial that they have been checked.
- Inform Program Director and Program Manager of any staff call outs, find coverage, lead staff will work vacant shift and/or shifts.
- Passes medications for staff who may be on boarding
- Ensure that staff is adhering to potential (pica), choking, fail risk, guidelines.
- Ensure that there is oral and written communication (completion of communication log and shift change reports) between the shifts.
- Ensure that staff are conducting clothing inventory on a monthly basis, completed by the 15th and ensuring that individuals receive all needed clothing via shopping, volunteer services and follow Medicaid pre-inspection guideline.
- Assist in grocery shopping and meal planning.
