Overview The Direct Support Professional will work under the supervision of the Program Coordinator. He/She will work with adults at a Center Based day program to build skills and achieve their personal goals in social and community related areas. Duties § Adhere to Governing Regulations Including Those Set Forth by Department of Behavioral Health and Developmental Services (DBHDS), Human Rights, Department of Medical Assistance Services (DMAS), Family Sharing's Home and Community Based Rights Policy, and Family Sharing, Inc. Personnel and Standard Operating Procedures. § Model Home and Community Based Services Rights for other employees, mentors, students and volunteers to include, but not limited to, community inclusiveness; facilitation of personal autonomy, independence and decision making; ensuring privacy, dignity, and respect; and encouragement of participation in service planning for all people receiving services from Family Sharing. Requirements Review Documentation for Accuracy Review Medication Transfer forms, Medication Bottles, MARs, Physician's Orders for Accuracy of Administration Interview the Person receiving supports Regarding Satisfaction with Provision of Services, Site Atmosphere and Environment, and Quality of Life § Facilitate and support Person receiving supports to access the greater community as much as they are able and willing to build social supports and engage in community life § Maintain Record of Mileage and Expenses § Maintain and Audit Service Files § Contribute to Annual Peer Performance Evaluation § Work with Support Coordinators, the Person receiving supports Family Members, Friends, and Other Providers of Support Services to Coordinate Cooperation of Service Delivery § Maintain a Daily Time Log of all Family Sharing, Inc.
