We are seeking a dedicated and compassionate Certified Home Care Aide to join our team of healthcare professionals at SYNERGY HomeCare Of East King County in Kent, Washington. As a HCA, you will play a vital role in providing high-quality care to our clients, promoting their independence, and enhancing their overall well-being.Responsibilities:
- Provide personal care services to clients, including bathing, dressing, grooming, and toileting
- Assist clients with daily living activities, such as meal preparation, medication management, and transportation to appointments
- Monitor and report any changes in clients' condition to the nursing team
- Develop and maintain positive relationships with clients and their families
- Maintain accurate and up-to-date records and documentation
- Collaborate with our interdisciplinary healthcare team to develop and implement individualized care plans
