Supporting our clients by helping them achieve the goals that they want to, to lead to the life they want. We are seeking Direct Support Providers to become an integral and valued part of our team. Direct Support Providers will be responsible for providing a wide range of services to clients depending on what those clients need support in. We do our best to accommodate all needs of employees in regards to schedule, disability and level of comfort.
Essential Duties and Responsibilities:
- Review Person Centered Support plans to ensure they are understood and implemented.
- Provide Direct Support to persons with varied disabilities.
This may include:
- Behavioral support that ensures the health, safety, and wellbeing of clients in their current placement and community.
- Support for making, planning and achieving goals.
- Engage in training and supportive services such as supervision to ensure maintenance and increase of skills needed to work with persons with disabilities.
- Creating a positive and supportive environment by directly providing behavioral support, encouraging positive choices, redirecting from challenging behaviors, and discovering/resolving underlying reasons for challenging behaviors.
- Ensuring that health care is readily accessible and accessed when necessary.
- Provide support to clients in maintaining and growing their connection to their community.
- Meet requirements for timely, accurate, and thorough documentation and required reporting.
- Attend and participate in staff meetings, supervision, and training as directed (including pre-service and annual training)
- Assisting clients with taking and tracking medications as well as completing training and certification for doing said tasks.
- Assisting clients with personal care as needed.
