Responsibilities Assist clients with Activities of Daily Living (ADLs) Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care plans following HIPAA guidelines and facility protocols Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary Maintain a clean and safe environment through regular cleaning duties and infection control practices Document patient observations, care activities, and communication with healthcare teams clearly and accurately Qualifications Prior experience working in group homes, assisted living, nursing homes, or home health care settings is highly preferred Knowledge of patient observation technique.
