  • Home Health Licensed Practical Nurse
  • in •Athens, AL.

  • Pay:
  • $33.00 - $36.00 per hour based on experience
  • Type:
  • Three-month contract
  • Schedule:
  • 32 hours guaranteed, 8:00AM - 4:30 (Monday-Friday)
  • Coverage Areas:
  • Limestone County
  • Primary Responsibilities:
  • Provide skilled nursing care to patients in their homes under physician orders.
  • Perform routine clinical tasks including medication reconciliation, wound care, vital signs, and patient assessments.
  • Accurately document care provided in the electronic medical record (EMR).
  • Educate patients and caregivers on disease management, medications, and care plans.
  • Communicate effectively with RNs, physicians, therapists, and the interdisciplinary care team.
  • Maintain compliance with state regulations, agency policies, and infection control standards.
  • Qualifications:
  • High School Diploma / GED
  • Active LPN license in Alabama
  • Experience in Home Health
  • Skilled in documentation
  • Wound care experience
  • Valid Driver's License
