About Us Crossville Health and Rehabilitation is dedicated to providing compassionate, high-quality long-term care and rehabilitation services. Our interdisciplinary team works together to help residents recover, regain independence, and enjoy the highest possible quality of life. Position Overview As a Licensed Practical Nurse (LPN) , you will provide direct nursing care to residents while collaborating with Registered Nurses, physicians, therapists, and other healthcare professionals. Your clinical expertise and compassionate approach help ensure residents receive exceptional care every shift. ✅ Key Responsibilities Administer medications and treatments as prescribed Monitor, assess, and document residents' health status, including vital signs Report changes in condition to the supervising RN or physician Assist residents with activities of daily living (ADLs) as needed Collaborate with the rehabilitation team to support recovery goals Maintain accurate documentation in accordance with facility policies and state/federal regulations Provide education and emotional support to residents and families Follow infection control and safety protocols to maintain a safe environment Qualifications Current and valid Licensed Practical Nurse (LPN) license Strong clinical skills, including medication administration and wound care Excellent communication and interpersonal skills Ability to work effectively in a team-oriented environment Compassionate, patient-focused approach to care BLS/CPR certification required Why Join Our PRN Team?
