Job Summary The Licensed Practical Nurse is a permanent, full-time position which is used by the wellness program. This is a position with the Bureau of Home and Community Services. The Bureau of Home and Community Services is the parent agency for the statewide Medicare/Medicaid Certified Home Health Program that serves all Alabama counties. Home health care services are provided for homebound patients who meet eligibility requirements. Responsibilities Enters referrals into HCHB and schedules visiting staff under directive or RN so that services are provided efficiently, with accuracy, as evidenced by medical record review, supervisory review and observation. Communicates professionally and appropriately with all Internal and external customers so that information is shared accurately, ensuring compliance with federal, state and agency policy as evidenced by medical record review, supervisory review and observation. Reviews and evaluates referral documentation and related documents, as assigned by, and under RN supervision, to determine if all required documentation is received, and confirms orders with signing doctor, prior to initiation of evaluation visit by RN. Provides administrative support by completing assigned tasks in a timely manner related to home care operations according to federal and state and agency policy as evidenced by supervisory review with no more than two valid complaints per year. Documents accurate data in the medical record with minimal supervisory guidance so that the information meets federal, state, and agency requirements, as evidenced by medical record review, supervisory review and observation. Responds to emergencies and natural disasters according to agency policy so that home care patients and citizens receive emergency and preventive health care with accuracy, as evidenced by medical record review, supervisory review and observation. Qualifications Graduation from an approved school of practical nurse education. Possession of a license as a practical nurse as issued by the Alabama Board of Nursing. $33,064.80 - $61,063.20 per year
