Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care, assisted living, hospitals, and community health environments. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, medication administration, and comprehensive assessments. Duties Perform detailed patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and specialized treatments such as botulinum toxin therapy. Manage complex care needs such as catheterization, tube feeding, ventilator support, and wound care using aseptic techniques. Collaborate with interdisciplinary teams to develop and implement personalized discharge planning and case management strategies. Document all patient interactions thoroughly within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Conduct triage assessments to prioritize urgent cases in emergency or urgent care settings. Assist with diagnostic evaluations including specimen collection/processing and medical imaging procedures when necessary. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable. Strong background in medical-surgical nursing, geriatrics, dementia care—including Alzheimer's and memory care—and hospice or palliative care. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation. Knowledge of
ICD-9/ICD-10
coding standards for medical records and billing purposes. Experience working with individuals with developmental disabilities or behavioral health needs is a plus. Proficiency in performing venipuncture, phlebotomy, injection administration, catheter care, and sterile processing techniques. Ability to handle high-pressure situations with excellent patient assessment skills and physiology knowledge. Commitment to maintaining HIPAA compliance and infection control standards while delivering compassionate patient care across diverse settings including outpatient clinics, home & community care environments or inpatient units.
