Job Description

Overview Join our dedicated healthcare team as a Licensed Practical Nurse (LPN), where your expertise will make a meaningful difference in patient care across diverse clinical settings. In this vital role, you will provide compassionate, skilled nursing support by performing patient assessments, administering medications, and assisting with treatments. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories, ensuring accurate documentation in electronic health record (EHR) systems such as Epic or Cerner. Administer medications, injections, and IV infusions following strict protocols and safety standards, including sterile techniques and aseptic procedures. Assist with medical procedures such as catheterization, wound suturing, spinal taps, and specimen collection/processing. Monitor patients' conditions closely, managing acute pain, airway management, ventilator support, and responding promptly to changes in patient status. Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans for patients with complex needs like dementia, Alzheimer's disease, or infectious diseases. Provide patient education on medication administration, nutrition, discharge planning, and health coaching tailored to diverse populations including pediatrics, geriatrics, and individuals with disabilities. Ensure compliance with HIPAA regulations and maintain accurate medical records while documenting all patient interactions thoroughly. Experience Valid licensure as a Licensed Practical Nurse (LPN) in the appropriate state or region. Proven experience working in various healthcare settings such as hospitals (including ICU or Level I Trauma Centers), nursing homes, assisted living facilities, or outpatient clinics. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for documentation purposes. Critical care experience preferred, including ICU or emergency room settings; experience with Level II trauma centers is highly valued. Familiarity with specialized care areas such as hospice care, palliative care, pediatrics (including neonatal ventilator management), or behavioral health is advantageous. Knowledge of ICD coding (ICD-9/10), CPT coding for procedures like Botox treatments or cardiac catheterization enhances your ability to deliver comprehensive patient care. Strong skills in patient assessment, medication administration—including injections and tube feeding—and infection control practices are essential for success in this role.
