Job Description

Active Licensed Practical Nurse (LPN) license and minimum 1+ years Home Health Licensed Practical Nurse (LPN) experience required. Schedule will be discussed during the interview process, allowing for a flexible approach to patient caseload management.
Qualifications:
Active and unrestricted LPN license required At least one year of recent experience in home health, preferably with geriatric patients Demonstrated proficiency with HomeCare HomeBase (HCHB) EMR system Solid clinical assessment, documentation, and patient communication skills Possess reliable transportation for patient home visits Strong ability to work independently and effectively manage caseloads
Key Responsibilities:
Deliver comprehensive nursing care in home settings as outlined in personalized care plans Carefully monitor patient status and update documentation using
HCHB EMR
Administer prescribed medications and treatments in compliance with state regulations Educate patients and families on managing conditions and preventive care strategies Collaborate with interdisciplinary healthcare teams to promote optimal patient outcomes
