Job Description

DUTIES OF POSITION
Assists in the coordination of client services under the direction of the Director of Nursing.
POSITION RESPONSIBILITIES
Identifies and evaluates client needs and appropriateness of personal care services. Upon receiving the referral for services, the intake nurse should make an initial phone call immediately. If unable to call immediately, an initial call should be made before the end of the same working day. Completes intake forms upon client admission. Notify Office Manager and Scheduling Team of completion of intake (Intake should be completed within 48 hours of receiving referral. Admission paperwork should be completed within 24 hours of intake). Communicates on a routine basis with Office Manager and Scheduling Team in regard to a client's specific needs and/or requests. Complete all intakes on Medicaid and/or PASSE clients discharged from hospital or facility within 48hrs of client returning home. Ensure all appropriate supporting documentation for re-assessment is completed and put into client's chart. Supervisory Home Visits are to be completed as follows: Medicaid/PASSE
  • Annually AR Choices
  • Every 6 months; rotating with aide present and not present. VA
  • Monthly or as Specified by Authorization Complete all PA paperwork for the initial visit as advised by Office Manager/DON.
Enter PA request to appropriate portal based on payer source and update Office Manager when request is entered. Develop the Care Plan after completing the initial assessment of the client & consulting the client and/or responsible party. Documents & reports changes in client status to Office Manager immediately. Review and update Care Plans per policy. Participates in community related functions per request of the Office Manager and/or marketing staff if schedule allows. Assist with PCA courses per request of DON The intake nurse must attend and participate in the weekly staff meeting. Performs other duties as requested by Office Manager, DON, and/or Director
QUALIFICATIONS
Currently licensed as an LPN or RN with the State of Arkansas in good standing. Has clear criminal background screening. Experience in a Home Care setting preferred. Must be computer literate and able maintain simple records.
JOB CONDITIONS
Position requires in office work 8:00am
  • 4:00pm, Monday-Friday Position is stressful in terms of meeting deadlines.
It is primarily a desk job which essentially involves sitting, standing, stooping, and walking, as well as, an inordinate amount of telephone communication. It requires minimal lifting of office records and printouts. The ability to read 12 point and larger type is required. One must be able to hear adequately on the telephone with no more than an amplifier and be able communicate both verbally and in writing in English. Position require travel to and from client homes as well as possible travel to in-service training.
EQUIPMENT OPERATION
The job requires the ability to utilize a PC, calculator, multi
