Medical terminology Full Job Description Job Summary We are seeking a dedicated and compassionate East Side PRN Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this role, you will provide high-quality patient care across various settings, including long-term care, assisted living, and community-based services. Your expertise will support patient assessments, medication administration, and vital signs monitoring, ensuring comfort and safety for diverse populations. This position offers flexible scheduling to meet the needs of our patients and the organization's mission to deliver exceptional care with energy and enthusiasm. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications, injections, IV infusions, and other treatments following established protocols while ensuring compliance with HIPAA regulations. Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing techniques. Support patients with dementia or Alzheimer's disease through memory care activities and behavioral health interventions. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients in acute care, long-term care, or home & community settings. Conduct patient triage over the phone or in person to determine urgency and appropriate level of care in urgent or emergency situations. Maintain accurate medical documentation, medical records, and coding (ICD-10/ICD-9) to ensure proper billing and compliance standards are met. Participate in discharge planning, case management, and patient education initiatives to promote optimal health outcomes. Skills Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for documentation and order entry. Critical care experience including ICU or Level I Trauma Center environments is highly desirable. Strong background in geriatrics, memory care, hospice care, or behavioral health enhances patient support capabilities. Knowledge of advanced nursing practices such as ventilator management, infection control protocols, and acute pain management. Skilled in performing venipuncture (phlebotomy), injection administration, catheter care, and specimen collection/processing. Familiarity with ICD coding (ICD-10/ICD-9), CPT coding for procedures like spinal taps or cardiac catheterization is a plus. Excellent patient assessment skills combined with physiology knowledge and medical terminology expertise. Ability to work effectively with individuals with developmental disabilities or disabilities requiring specialized nursing interventions. Experience in emergency medicine or urgent care settings is advantageous for handling acute situations swiftly. Join us in making a meaningful difference every day!
