As a Licensed Practical Nurse (LPN) , you'll provide hands-on patient care under the guidance of a Registered Nurse, ensuring comfort, safety, and positive outcomes for patients in their homes. Make a real impact in by providing care that improves lives every single day Key Responsibilities Deliver compassionate, direct patient care in accordance with state regulations Implement care plans developed by the RN and document progress accurately Monitor patient conditions and communicate observations to the RN and physician Collaborate with the care team to ensure seamless, high-quality service Participate in professional development and team meetings Requirements Must be a current Licensed Practical Nurse (LPN) in the State of Arizona 3 years of nursing experience preferred (Home Health or MedSurg a plus) Current CPR certification Valid AZ driver's license, auto insurance, and reliable transportation The employer for this position is stated in the job posting.
