Fingerprint Clearance Card is Required CPR Must currently be in Arizona/greater Phoenix area Job responsibilities: Conduct comprehensive head-to-toe assessments, record vital signs, monitor changes in physical and mental conditions, and document all findings in medical records. Administer medications as prescribed by physicians, and monitor residents for any adverse reactions or side effects. Collaborate with an interdisciplinary team (physicians, physical therapists, dietitians, social workers, etc.) to develop, implement, and evaluate individualized care plans for each resident. Act as a liaison between residents, families, and the healthcare team, ensuring all parties are informed of health status changes and the resident's wishes are respected. Maintain accurate and detailed patient charts and records to ensure continuity of care and compliance with all regulations. Respond promptly to medical emergencies, such as falls or sudden declines in condition, and provide immediate interventions. Welcome residents and orient them to the environment.
