We are seeking a motivated and compassionate Licensed Practical Nurse (LPN) to join our team in providing private duty, in-home care on a full-time basis. This vital role involves delivering personalized nursing care directly in clients' homes, ensuring their health, safety, and comfort. Responsibilities Monitor vital signs, perform physical examinations, and assess client conditions regularly Report any changes in patient's condition to the office with the reports documented in patient files. Maintains excellent communication with patients, and staff. Perform medication reminders to clients. Assist with tube feedings and daily outings Qualifications Valid LPN license in good standing with relevant state licensing boards Critical care experience or background working in hospital settings such as ICU or trauma centers is highly preferred Experience working with individuals with developmental disabilities, Alzheimer's disease, dementia, or memory care needs Strong knowledge of physiology, anatomy, infection control practices.
