Overview The LVN will work under the direction of the DON and Charge RN to provide individualized nursing care to SNF residents and acute/swing-bed patients. The role includes patient assessment, medication administration, treatments, documentation, patient/family education, emergency response, and coordination with physicians and other healthcare departments. Duties Perform patient assessments and report abnormal findings to the Charge Nurse/physician. Develop and follow individualized plans of care . Administer medications and treatments accurately and safely. Perform nursing procedures such as EKGs, blood draws, IV starts, wound care, and specimen collection . Monitor and document patient condition, nursing interventions, and progress. Respond appropriately to emergency situations and initiate CPR/resuscitation protocols when required. Provide patient and family education. Maintain infection control, patient safety, confidentiality, and accurate documentation. Assist CNAs and other nursing staff with direct patient care as needed. Participate in shift-to-shift handoff and walking rounds. Assist with orientation and education of new staff. Maintain supplies, equipment, treatment areas, and patient rooms. Coordinate patient transfers for procedures and diagnostic testing. Attend required staff meetings, CPR training, and in-service education. Requirements Active California LVN/LPN license — must be currently active.

BLS/BCLS

certification required. Minimum 1 year of experience . SNF/Skilled Nursing experience preferred based on the specialty.

Pay:

$1,800.00 - $2,100.00 per week
