In this vital role, you will provide compassionate patient care across various settings, including acute care, long-term care, assisted living, and community health environments. Your expertise will help ensure optimal patient outcomes through comprehensive assessments, medication administration, and collaborative care planning. This position offers an exciting opportunity to work with diverse populations, including individuals with developmental disabilities, dementia, and complex medical needs. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations using EMR (Electronic Medical Records) systems such as Epic or Cerner. Administer medications and injections accurately following HIPAA regulations and established protocols; perform IV infusions and catheterizations as needed. Manage airway support and ventilator care for patients requiring respiratory assistance; utilize aseptic techniques during sterile procedures like suturing and specimen collection. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients across settings such as hospitals, nursing homes, hospice, and home care. Monitor patients' conditions closely during emergency situations like acute pain episodes or trauma cases; perform triage assessments in urgent care or emergency room environments. Document all patient interactions thoroughly in medical records, ensuring compliance with
ICD-9/ICD-10
coding standards for accurate billing and reporting. Educate patients and their families on health coaching topics including medication management, nutrition, dementia care, and discharge planning. Qualifications Valid LVN or RN license in good standing; current CPR/BLS certification required. Proven experience working in critical care settings such as ICU, emergency medicine, Level I/II trauma centers or similar high-acuity environments. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with ICD coding (ICD-9/ICD-10). Experience providing specialized care such as Alzheimer's/dementia/memory care, hospice services, pediatric or neonatal ventilator management preferred. Strong knowledge of medical terminology, anatomy, physiology and diagnostic evaluation techniques. Ability to perform venipuncture, phlebotomy, tube feeding administration and sterile procedures confidently. Excellent patient assessment skills coupled with critical thinking to prioritize urgent needs effectively.
