Job Description

Overview We service sub-acute care level patients. Experience with Sub-acute level of patients and Vent/trach certification are preferred. IV Certification is preferred. LVN ( Licensed Vocational Nurse): Provides direct patient care and implements a plan of care initiated by the registered nurse. Provides accurate and timely documentation consistent with the plan of care. Performs direct nursing care including skin, bowel, bladder programs, nutrition ,hydration, medications and ordered treatments, transfers, NG tube and gastrostomy tube feedings. Knowledge of assessment and documentation, spinal cord injury, wound care and staging, g-tube placement and care, tracheostomy care and ventilator experience. Positioning and wheelchair safety. Uses equipment and supplies effectively and efficiently. Participates in patient and family education. Reporting the identified needs for other disciplines (OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor. Utilizes all available resources in planning care and consults as needed with RN's, medical staff, outside specialists or consultants, and other members of the interdisciplinary team. Continuously reviews, evaluates, and documents the effectiveness of nursing care, and accomplishment of client goals for discharge. Performs other duties as assigned by the registered nurse and/or Clinical Supervisor. 12 hr shift days, nights and weekends availability Compensation is based on experience.
