Compensation:
$40 - $45 hourly
Responsibilities:
Provide compassionate, patient-centered care in the comfort of patients' homes, ensuring their safety and well-being. Administer medications and treatments as prescribed, monitoring patients' responses and reporting any changes to the healthcare team. Collaborate with patients, families, and the healthcare team to develop and implement personalized care plans. Educate patients and their families on health management and disease prevention, empowering them to take charge of their health. Document all patient interactions and care provided accurately and promptly, maintaining confidentiality and compliance with regulations. Conduct regular assessments of patients' health status, adjusting care plans as needed to promote optimal outcomes. Communicate effectively with the healthcare team to ensure continuity of care and address any concerns or changes in patient condition promptly.
Qualifications:
Experience in providing patient-centered care in home health settings. Ability to administer medications and treatments accurately and safely. Proven track record of collaborating effectively with healthcare teams and families. Strong communication skills to educate patients and families on health management. Proficiency in documenting patient interactions and maintaining confidentiality. Ability to conduct thorough health assessments and adjust care plans accordingly. Licensed Practical Nurse (LPN) certification in the state of California required. About Company At
