In this vital role, you will provide compassionate nursing support across various settings, including acute care, long-term care, assisted living, and community health environments. Your skills in patient assessment, medication administration, and clinical procedures will help ensure the highest standards of care are maintained. We are looking for energetic, detail-oriented professionals eager to make a difference in patients' lives through expert nursing practice and compassionate service. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders, including IV infusion and catheterization procedures. Assist with medical procedures such as wound care, suturing, tube feeding, and sterile processing techniques. Monitor patients' conditions closely, utilizing critical care experience or ICU background to identify changes in status promptly. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with diverse needs such as dementia, Alzheimer's disease, or developmental disabilities. Conduct diagnostic evaluations like laboratory specimen collection/processing and basic medical imaging support when required. Maintain detailed medical documentation ensuring compliance with HIPAA regulations and ICD coding standards (ICD-9/ICD-10). Provide patient education on health coaching topics including medication management, nutrition, discharge planning, and chronic disease management. Support emergency response efforts by managing airway management, ventilator use, or acute pain management protocols in urgent or hospital settings such as Level I Trauma Centers or Emergency Rooms. Requirements Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Proven experience in diverse clinical environments such as hospital medicine, medical-surgical units, ICU, emergency medicine, or long-term care facilities like nursing homes or assisted living communities. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to patient assessment and treatment procedures. Experience with specialized areas such as hospice care, geriatrics, pediatrics (including neonatal ventilator), infectious disease care, or behavioral health is highly desirable. Ability to perform venipuncture, phlebotomy, injection administration, catheter care, and sterile techniques confidently. Excellent communication skills to effectively triage patient needs and collaborate with healthcare teams across multiple disciplines.
