In this vital role, you will provide high-quality patient care across various settings, including long-term care, assisted living, hospital environments, and community health when necessary. Your expertise in nursing practices, patient assessment, medication administration, and specialized care such as dementia, Alzheimer's, and pain management will ensure our patients receive personalized and effective treatment. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations while working in a fast-paced, supportive environment. Will be at office most of the time overseeing medical paperwork. Duties Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient needs using physiology knowledge and medical terminology. Oversee all medical paperwork and be able to order and manage all medical necesity for hom health patients. Administer medications and injections accurately following established protocols, including IV infusions and catheter care. Manage complex cases involving acute pain, dementia, Alzheimer's disease, infectious diseases, and trauma care. Operate electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth to document patient information meticulously and ensure compliance with HIPAA regulations. Assist with diagnostic evaluations including specimen collection/processing, medical imaging coordination, and laboratory procedures. Support discharge planning by coordinating with case management teams to ensure seamless transitions from inpatient to outpatient or home care services. Conduct critical thinking skills to prioritize urgent needs effectively. Deliver compassionate memory care for patients with dementia or behavioral health challenges while working with individuals with developmental disabilities. Perform sterile techniques during procedures such as wound care, picc line and g-tube cleaning and dressing change. Educate patients and families on health coaching topics including nutrition, medication management, and post-acute care instructions. Maintain accurate medical documentation of intake assessments, specimen collection/processing, CPT coding for billing purposes, and medical records updates. Qualifications Valid LVN license in good standing with the state licensing board. Atleast 1 year of experience in Home Healthcare Setting. Proven experience in diverse healthcare environments such as, nursing homes, assisted living facilities, or home & community care settings. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; proficiency in
ICD-9/10
coding is advantageous. Strong knowledge of medical terminology, anatomy, physiology, infection control practices, and HIPAA regulations. Experience administering injections, phlebotomy (venipuncture), catheterization procedures, tube feeding management, and ventilator support highly desirable. Ability to perform physical examinations and diagnostic evaluations accurately while demonstrating excellent patient assessment skills. Skilled in medication administration including acute pain management protocols; Exceptional communication skills to work effectively with multidisciplinary teams and diverse patient populations including pediatrics and geriatrics. Certification in Basic Life Support (BLS) required; additional certifications such as ACLS or PALS are a plus.
