Orange County and surrounding areas About IntelliCare Home Health Agency IntelliCare Home Health Agency is an ACHC-accredited and Medicare-certified home health agency dedicated to delivering compassionate, patient-centered care in the comfort of our patients' homes. We are seeking dependable, compassionate, and skilled Licensed Vocational Nurses (LVNs) to join our growing team on a per diem basis. Position Summary The Home Health LVN provides skilled nursing care to patients under the supervision of a Registered Nurse (RN) and in accordance with the physician's orders, individualized Plan of Care, Medicare regulations, and agency policies. The LVN works collaboratively with the interdisciplinary team to promote patient safety, improve health outcomes, and help patients remain safely at home. Responsibilities Provide skilled nursing visits as assigned by the supervising RN. Perform focused patient assessments and report changes in condition promptly to the RN and physician. Administer prescribed medications and treatments within the LVN scope of practice. Perform wound care and dressing changes as ordered. Provide catheter care, ostomy care, and other skilled nursing procedures. Monitor vital signs, pain levels, blood glucose, and overall patient status. Reinforce patient and caregiver education regarding medications, disease management, infection prevention, fall prevention, nutrition, and home safety. Observe for signs of complications and communicate significant findings to the supervising RN. Maintain accurate, timely, and compliant documentation in the agency's electronic medical record (EMR). Follow infection control practices and safety standards. Collaborate with physicians, Registered Nurses, therapists, and other members of the interdisciplinary team. Maintain compliance with Medicare regulations, ACHC standards, HIPAA requirements, and agency policies. Qualifications Current, unrestricted California Licensed Vocational Nurse (LVN) license. Minimum one (1) year of LVN experience; home health experience preferred. Current CPR/BLS certification. Valid California driver's license, reliable transportation, and current automobile insurance. Ability to travel throughout the assigned service area. Strong communication, organizational, and documentation skills. Ability to work independently while collaborating with the supervising RN and healthcare team. Experience with electronic medical records (EMR) preferred. Preferred Qualifications Home health or hospice experience. Wound care experience. Knowledge of Medicare home health regulations. Experience using home health EMR systems.
