Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs. Provide education on disease management, medication administration, and health coaching for patients and their families. Requirements Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).
