We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes. This per diem position offers flexible scheduling and the opportunity to make a meaningful impact on patient care.

Responsibilities:

Provide skilled nursing care per physician orders and plan of care Administer medications, perform wound care, and monitor patient status Document care accurately and report changes to supervising RN Educate patients and caregivers on treatments and safety Follow all agency policies and infection control standards

Qualifications:

Active LVN license in the state of practice CPR/BLS certification Home health experience preferred Valid driver's license and reliable transportation Strong communication and time-management skills

Pay:

$50.00 per hour
