Job Overview We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to provide exceptional, patient-centered care in a home health setting. The ideal candidate will have at least one year of experience delivering high-quality nursing care and demonstrate a strong commitment to supporting patients and their families with professionalism, empathy, and clinical excellence. This position involves caring for a 19-year-old female patient who requires total care assistance , including management of a gastrostomy tube (G-tube), tracheostomy, and mechanical ventilator . The LVN will be responsible for monitoring the patient's condition, administering treatments as prescribed, maintaining accurate documentation, and ensuring a safe and comfortable home environment. Responsibilities Provide comprehensive nursing care for a medically complex patient in the home setting. Manage and monitor G-tube feedings, tracheostomy care, and ventilator support. Assess and document patient status, vital signs, and any changes in condition. Administer medications and treatments according to physician orders. Collaborate with family members and the healthcare team to ensure continuity of care. Maintain a safe, clean, and supportive environment while promoting patient comfort and dignity. Qualifications Current and active California Licensed Vocational Nurse (LVN) license. Minimum of one year of nursing experience, preferably in home health, private duty, pediatric, or long-term care settings. Experience with tracheostomy, ventilator, and G-tube care preferred. Strong clinical assessment, communication, and documentation skills. Compassionate, reliable, and dedicated to delivering high-quality patient care.
