  • Proficient in EHR systems
  • Strong patient care and monitoring skills
  • Knowledge of medical terminology
  • Experience in discharge planning
  • Excellent communication skills
  • Ability to work in home health setting
  • Familiarity with catheterization and tube feeding
  • Basic life support certification from the American Heart Association or American Red Cross
  • Understanding of HIPAA regulations
  • Must have tracheostomy and catheter experience Responsibilities
  • Provide direct patient care in a homecare setting, focusing on individualized treatment plans and compassionate support.
  • Conduct patient assessments, including catheterization, tube feeding, and monitoring vital signs.
  • Collaborate with healthcare teams to ensure optimal patient outcomes and continuity of care.
  • Utilize EMR systems for accurate documentation and communication.
  • Implement discharge planning strategies to facilitate smooth transitions for patients.
  • Apply medical terminology and nursing expertise to deliver high-quality care tailored to each patient's needs.
Pay:

$28.00
