Overview Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) specializing in Home Health Care! In this vital role, you will deliver compassionate, high-quality nursing services directly to patients in their homes, promoting health, independence, and well-being. Your expertise will support individuals across diverse settings, including long-term care facilities, assisted living communities, and acute care environments. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a dynamic and supportive environment. Responsibilities Provide comprehensive patient assessments, including vital signs, physical examinations, and medication administration using sterile techniques and medical terminology. Administer treatments such as IV infusions, injections, catheter care, and tube feeding while ensuring adherence to HIPAA regulations and infection control protocols. Document patient care accurately within electronic health record (EHR) systems like Epic, Cerner, or Athenahealth; maintain detailed medical records and medical documentation. Assist with diagnostic evaluations including specimen collection/processing and basic medical imaging procedures when necessary. Educate patients and their families on disease management, medication use, nutrition, and post-discharge care plans to promote health coaching and self-care. Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of home & community care services for individuals with disabilities or complex needs such as dementia or Alzheimer's care. Respond promptly to triage calls or urgent situations involving acute pain management or infectious disease concerns; utilize critical care experience and emergency medicine skills as needed. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in the state of practice. Proven experience in diverse healthcare settings such as hospital medicine, ICU, emergency room, nursing home, hospice care, or post-acute care environments. Familiarity with EMR/EHR systems including Epic, Cerner, eClinicalWorks or Athenahealth for accurate documentation. Knowledge of
