Monitor patient conditions and promptly communicate changes to the clinical team. Administer medications and treatments as ordered. Reinforce patient education and individualized care plans. Complete accurate and timely electronic documentation. Collaborate with physicians, RNs, therapists, and other healthcare professionals to ensure excellent patient outcomes. Build lasting relationships with patients and their families through compassionate, quality care. Qualifications Current California LVN license in good standing Graduate of an accredited vocational nursing program Minimum of one (1) year of nursing experience (Home Health experience preferred) Current CPR certification IV/Blood Draw Certification preferred Valid California driver's license with a clean driving record Strong communication, organizational, and time management skills Spanish-speaking preferred Service Area This position primarily serves patients throughout: North Salinas Prunedale Aromas Surrounding communities throughout Northern Monterey County Our goal is to schedule clinicians within their assigned service area whenever possible to promote efficient travel and continuity of patient care.
