Qualifications LPN Full Job Description Location:
East Bay Area. Flexible territory assignments available, with patient visits coordinated around your preferred coverage area and proximity to home whenever possible. Job Summary We are seeking a Licensed Vocational Nurse (LVN) to deliver compassionate, patient-centered nursing care within the home setting. This role involves assessing and observing patients and/or caregivers to enhance their quality of life and providing individualized education. The LVN will adhere to nursing policies and procedures, follow physician-directed care plans, and uphold Medicare and Medicaid regulations to ensure high standards of patient care. Qualifications/Educational Requirements Current Licensed Practical Nurse (LPN) licensure. Minimum of 1 year of experience in a healthcare setting. Strong oral and written communication skills. Key Responsibilities Collaborate effectively with the healthcare team to ensure seamless patient care. Utilize strong organizational and time-management skills. Support quality improvement initiatives and strive to uphold agency standards. Conduct nursing procedures in alignment with agency protocols. Provide care under the supervision of a Registered Nurse (RN). Monitor and document patient reactions and progress through observation, assessment, and evaluation. Educate patients and family members on disease management, medications, treatment options, and home care procedures as outlined in the plan of care. Report any adverse findings promptly to the physician and RN. Follow Medicare and Medicaid guidelines for patient care delivery. Coordinate and oversee patient care services to meet individual needs. Comply with HIPAA regulations and ensure confidentiality both inside and outside the office. Adhere to infection control protocols in all patient interactions. Document each skilled visit according to Medicare standards. Maintain accurate patient records following agency policies and procedures. Participate in ongoing training, in-services, and professional development annually. Perform other duties as assigned, supporting overall patient care and agency goals. Ability to lift up to 50 lbs. Hourly Range :
$40.00 - $48.00, commensurate with experience and qualifications.
