Part-Time MDS Nurse (LVN) San Bruno Skilled Nursing is seeking a knowledgeable and detail-oriented MDS Nurse (LVN) to join our team on a part-time basis. Ensure clinical documentation accurately reflects resident conditions and supports reimbursement processes. Maintain compliance with CMS, state, and facility regulations. Collaborate with the interdisciplinary team to develop and update individualized care plans. Monitor resident progress and ensure care plan revisions are completed as needed. Participate in care conferences and quality assurance activities. Ensure timely and accurate MDS submissions for Medicare and Medicaid reimbursement. Assist during audits, surveys, and regulatory reviews. Provide education and guidance to staff regarding documentation standards and MDS requirements. Communicate effectively with residents, families, and healthcare team members regarding care planning and resident needs. Stay informed of regulatory updates affecting MDS processes and skilled nursing documentation. Qualifications Current California LVN license in good standing. Previous experience in skilled nursing, post-acute care, or long-term care preferred. Working knowledge of MDS 3.0 , the RAI process , and Medicare/Medicaid reimbursement guidelines. Strong organizational, analytical, and documentation skills. Ability to manage multiple deadlines while maintaining accuracy and compliance. Experience with electronic health record (EHR) systems. Proficiency in Microsoft Office applications. Strong communication and collaboration skills.
